Your initial appointment will consist of a consultation explaining your diagnosis, treatment options, risks & benefits, and answer any and all questions you may have.
Most insurance companies require prior approval for treatment plans. Your consultation appointment will determine your treatment plan for insurance analysis and estimate of covered benefits.
Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip and any x-rays from your dentist.
- A list of medications you are presently taking.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
- You must bring a current driver’s license or passport for appropriate identification.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or court appointed guardian at all appointments.
Please have nothing to eat or drink after midnight prior to your surgery. A minimum of six hours with nothing to eat or drink (including water) will be required or surgery will be rescheduled. If your surgeon informed you to take medicines in the morning before your surgery (antibiotics, blood pressure or acid reflux medications), only drink enough WATER to swallow the pill at least one hour prior to your appointment, You will also need an adult to drive you home; they will need to be present prior to starting surgery and will need to remain at our office during the entire surgery so you may leave with them as soon as you recovered.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, history of bacterial endocarditis, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy coumadin or warfarin, etc.). Bring a current list of all of your medications with you. Surgery may need to be scheduled pending test results or medical recommendations from your doctor and our Surgeon. Your safety during surgery is our most important concern.
If your Dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary they can be taken at our facility. If you bring your x-rays with you this will defer any cost that we may have to charge for these studies. Our Surgeon will need to see the X-Ray at the consultation even if they are duplicates or studies your dentist or physician has completed.
